• All minors are required to have their information filled out by a parent or legal guardian. Our office will confirm the information and status of the legal guardian or parent before confirming the appointment.
• If you need to cancel or reschedule your appointment, you will need to call Our Office directly. Cancellation or rescheduling an appointment within 24 hours of the appointment time may incur a $25.00 fee.
• You agree to receive e-mail, text message, and phone calls to confirm your scheduled appointment.
