- The information you provide when scheduling an appointment through our website will be saved to your or your family’s account in Our Office.
- All minors are required to have their information filled out by a parent or legal guardian. Our office will confirm the information and status of the legal guardian or parent before confirming the appointment.
- If you need to cancel or reschedule your appointment, you will need to call Our Office directly. Cancellation or rescheduling an appointment within 24 hours of the appointment time may incur a $25.00 fee.
- You agree to receive e-mail, text message, and phone calls to confirm your scheduled appointment.
Payments & Services
Fees for services will be communicated before or during your consultation. Payment terms may vary depending on treatment plans and insurance coverage.
